Project #22831 - Literature Review-Action Research

Complete a review of literature for your action research project.

Your review of literature should include a summary of at least eight sources that can be used as references. At least five of the sources must be from peer-reviewed journals.

For each resource, you should also include an explanation of how it supports or relates to your action research project.

Prepare this assignment according to the APA guidelines found in the APA Style Guide, located in the Student Writing Center. An abstract is not required.

 

My Project Subject is-Reducing Hospital Admissions/Visits of patients with Chronic Obstructive Pulmonary Disease (COPD) with home follow up after hospital discharge.

Here is my Problem Statement:

                                                               Action Research Problem:

 Many patients who suffer from Chronic Obstructive Pulmonary Disease (COPD) are like a revolving door at Piedmont Medical Center would it be beneficial to implement a follow up program for these patients when discharged? Will follow up with their home regimen and compliance with physician ordered medications help reduce hospital visits?

The repeated visits are a heavy financial burden for the hospital, insurance company and the patient. These frequent hospital visits also increases the patients risk for acquiring a nosocomial infection or pneumonia, decreases the patient’s quality of life and can also be a heavy burden on the patient’s family and friend support system.  

¡ Across a selection of 15 states in 2008, there were 190,700 index admissions with a principal diagnosis of COPD among patients 40 years old and older. The 30-day readmission rate for a principal diagnosis of COPD was 7.1 percent and the rate of 30-day readmissions for any diagnosis of COPD was 17.3 percent.

¡ If all readmissions were counted regardless of diagnosis, one out of five index admissions for COPD (20.5 percent) were followed by a readmission within 30 days.
