Project #61769 - Defining Priorities - Public Health

Using the space provided, answer the following questions. You are to write on this document – you need to write neatly as anything I can’t read will be considered incorrect. The reason for writing on the document (and not typing) is so that you stay focused on answering the question at hand. Please do not write more than I ask for – if it asks for a short answer (list/name/identify), do that and no more. Read each question very carefully to ensure that you are providing the correct answer. This assignment is due Friday March 13, 2015; no late assignments will be accepted due to spring break.

Program: This fall, the University will be hiring a Sustainability Coordinator (SC) who will be responsible for many environmental issues on campus, including reducing our carbon footprint. In order to do so, it is important to address recycling on campus and you have been hired to help the SC by creating a program aimed specifically at the residence halls. This program will have multiple components, including educational programs in the halls as well as contests and recycling drives as promotional components. This is to be a one-year program with planning time beginning on July 1, with activities beginning at the start of fall semester and ending around the end of spring semester. Your project year ends June 30 to allow for evaluation time.

In order to start designing your program, you need to find your focus. This requires a mission statement, goals and objectives.

1. Mission statement (3 points)

2. Next you need one goal and three corresponding SMART objectives. You should have one objective that addresses a change in participant knowledge, one addressing change in participant behavior, and one that addresses change in participant attitude/belief that matches with the program goal. Make sure you match the objective to the right topic. (2 points each)

a. Goal:

b. Objective #1 (knowledge):

c. Objective #2 (behavior):

d. Objective #3 (attitude/belief):

3. The next step is to identify some strategies to meet your goals and objectives. Given that it is a year-long program, using a variety of implementation strategies will be the most effective. You are to pick three strategies – one health education and two others of your choice (but NOT incentives). Be sure to label which category the two strategies of your choice are and make sure they are all from different categories. Do not just list the strategy; make sure you briefly describe it as it applies to your program. You want to make sure that I can see how the activity fits as a component of your program. (2 points each)

a. Health education strategy:

b. Strategy #2, Type/Name: ___________________________

c. Strategy #3, Type/Name: ___________________________

4. With most program activities, incentives can be helpful to encouraging participation. It is important to match the incentive with the program activities and also use a variety of incentives. For your program, name/list two incentives you will use in your program, including a brief one sentence description of why you choose that specific incentive. (4 points)

5. The next step for you is to create a Logic Model that will help guide your process. Using the attached form, you are to fill out the logic model. For each category, you are to put the number of items in a category as is stated in the column headers. (15 points total) [Logic Model is on page 4]

6. Since this is a yearlong project, you will need to create a timeline as you prepare to begin the design of the project. Use the information in the logic model to help you map out your year of activities. You will need to have eight items in the timeline – make sure you match the starting month of your program with the description. Make sure that you consider your implementation timeframe when designing the timeline. You should have planning, implementation and evaluation activities in your timeline. It is important that you use your own words and NOT the timeline from the book. (8 points) [Timeline is on page 5]

7. There are many implementation concerns related to this program its corresponding activities. Using the following subjects, tell me one specific problem that might arise for each category and how you will address it – make sure it fits with the program topic. (2 points each)

a. Medical/safety:

b. Record keeping:

c. Staff training:
