Introduction

Now that you are familiar with Threat and Error Management (TEM) and its useful application in Crew Resource Management (CRM) discussion, you will have the opportunity to apply it to one commonly cited accident in human factors literature. Air Florida Flight 90 (callsign Palm 90) was en route from Washington National Airport in Arlington, Virginia to Ft. Lauderdale Hollywood International in Ft. Lauderdale, Florida on January 13, 1982 when the aircraft stalled and crashed into the 14th Street Bridge in Washington, D.C. It was later determined that the aircraft’s wings were significantly contaminated with snow and ice and the engine power improperly set for takeoff power.

Instructions

First, research the Air Florida Flight 90 accident keeping TEM in mind. There are plenty of resources available regarding this well-known accident. However, one that may prove useful is:

This is a simple exercise which helps you to identify several examples of threats, errors, and Undesired Aircraft States (UAS) present in the Air Florida accident.For each component of TEM – threats, errors, and UAS provide at minimum two examples. You will then provide a short narrative about the result of that threat, error, or UAS in terms of the accident chain. This outcome may be an error, incident, accident, remains unchanged, etc.

Grading

This is an identification and fill-in-the-blank assignment. The student will be issued full credit for the assignment if he/she:

  • Completes the TEM worksheet and submits by the due date
  • Provides at least two examples of threats, errors, and UAS (six examples total)
  • Provides a short narrative regarding the outcome of the threat, error, or UAS in terms of the error chain
